To help your visit go smoothly, please bring the following with you:

Insurance card
Valid ID
Medical history, including a list of current medications
Patient forms (completed online or filled out in the office)

What Happens During The Visit

Check-In & Paperwork

When you arrive, our front desk team will welcome you and help you complete any remaining forms. If you haven’t filled them out beforehand, you can do so in the office.

Comprehensive Exam & Imaging

Your appointment may include X-rays, a cleaning, and a thorough exam. The doctor will evaluate your oral health and discuss any findings with you in a clear, straightforward way.
